Services We Provide
The Program Director/CEO will be responsible for completing intake and admission/assessment information on individuals being considered for the program.
The Program Director/CEO will be responsible for development of the Individualized Service Plans. The Service Plan Coordinator will be responsible for reviewing and approving (by signing) the ISPs completed by the Training Specialist prior to delivery of services.
The Program Director/CEO will provide training, supervision and supports to individuals in the following areas:
The Program Director/CEO will be responsible for monitoring of services provided and sharing information with the individual’s primary case manager regarding psychiatric instability, follow-up with medical care needed, and referrals to community agencies for personal items.

The Program Director/CEO will provide training, supervision and supports to individuals in the following areas:
- Housekeeping Skills – Train, support and assist in common household related tasks in the areas of lunch preparation/set-up, meal clean up, vacuuming,
- Social/Communication Skills – Train and assist each individual in developing acceptable and appropriate ways of expressing feelings, managing impulses and relating to others. Includes as needed, training in social mores, sexuality and use of alcohol and tobacco.
- Academic Skills – Train in functional academics to include writing name, time and place orientation, simple math concepts, recognition of survival words or signs, use of the telephone, emergency procedures, money management relaying personal data, etc.
- Community Skills/Community Safety - Provide community based experiences to acclimate consumers to community opportunities and how to use them appropriately.
- Leisure and Recreational Skills – Provide individuals and opportunity to experience and participate with in the mainstream of the community and at the center in a variety of recreational and leisure activities.
- Health and Safety – Train and assist individuals to identify unsafe situations and to seek medical attention when illness occurs. Health and Safety – Train and assist individuals to identify unsafe situations and to seek medical attention when illness occurs.
- Sensory stimulation and gross/fine motor skills- introduction to activities that will strengthen gross/fine motor functioning and sensory stimulation.
- Behavior /Problem Solving Skills – integrate problems solving techniques into training activities as issues arise and teach alternatives to negative behaviors. Implement Behavior Plan developed by qualified professionals, as needed.

Our Offer
- Safe and caring home 24 hours per day seven days per week.
- Case Management Services
- Monitoring of all medical needs (utilizing community health and mental health providers), including the prescribing, adjusting and administration of psychotropic medications.
- Enrollment in an appropriate private school, and ongoing interaction with school personnel to monitor the resident’s adjustment.
- Ongoing contact with the custodial agency to review progress in the consumer service plan.
- Immediate notification of the referring agency in the event of medical emergencies, significant behavioral emergencies, or unauthorized leave from the facility.
- Monthly written and quarterly reports to the referring agency summarizing the resident’s progress in achieving his/her goals
Admission Criteria
- Must have a psychological approved for Intellectual Disability waiver services.
- Adult Male and female 18 years and older with Intellectual Disability Mild to Profound regardless of race, sex, religion and background. These individual’s may also be dually diagnosed with a physical or mental disorder.
- Approve for MR waiver services.
- Can be ambulatory or non-ambulatory.
- Complete an admission application.
- Have a current assessment, indicating needs.
- Medical and social history.
- Current physical examination report and TB screening results.
- Face to-Face Interview.
The full admission packet will be discussed upon approval of admission.

Exclusionary Criteria
- Individuals who have skilled nursing needs
- Individuals diagnosed with a communicable disease
- Individuals who present as an imminent danger to self and others
- Individual whose has exhibited actions that indicate him/her to be a danger to self or others, within the past 6 months.
- Individual under the age of 18 years.
Financial
- Bridgeway Residential will be reimbursed in accordance to Medicaid Waiver Rates.
- Room and board is paid by individual.
- Bridgeway Residential Services accepts private pay and rates are the same as Medicaid Waiver Rates
- Medical assistance will cover health care cost or other insurance provided by the custodial agency or the consumer’s parent (s) or guardian.
Intake Procedure
- A program application is completed by the individual or by a family member or support coordinator on behalf of the consumer and returned to the Program Director/CEO upon completion.
- A current copy of the individual’s physical must accompany the application.
- A copy of the individual’s psychological evaluation must also accompany the application.
- A face to face interview with the individual, family member and/or support coordinator will be arranged by the Program Director or CEO Assessment may be initiated during this meeting or when services first begin by Program Director or CEO .
- A staffing will be coordinated and facilitated by the individual’s case manager to include the individuals, staff from the program and any family members. This meeting will help identify the individual needs and aid in the development of the Individualized Service Plan (ISP). The Program Service Agreement is signed at this time.
- Development of the ISP and completion of the ISAR will be done by the Program Director, or CEO.
- Submission of the ISAR and ISP will be done by Program Director or assigned program staff to the individual’s support coordinator to submit to the DBHDS for authorization of services.
Continued Stay
An Individual must continue to meet 2 of 7 areas on the Level of Function assessment completed by his/her case manager and have a continued need for training.
Discharge from Service
Discharge from services will be determined under the following circumstances:
Individuals discharged form the program will be notified of their right to appeal in writing at least 10 days in advance.
- Individual has met or exceeded expectations of the program
- Individual no longer meets the criteria to receive Waiver services
Individuals discharged form the program will be notified of their right to appeal in writing at least 10 days in advance.
Termination of services
Termination of services will be determined under the following circumstances:
Individuals terminated form the program will be notified of their termination and right to appeal in writing at least 10 days in advance unless they are a danger to themselves or others.
- Individual no longer is happy with his or her services and request termination
- Individual is excessively destroying property
- Individual has a substance abuse problem which requires treatment that we are unable to provide
- Individual needs constant supervised medical treatment
- Individual refuses to take prescribed medication for medical problems or psychiatric stabilization
